The Immunization Clinic is committed to quality health service delivery and provides the following services to promote prevention, intervention, and protection:

IMMUNIZATIONS
Children and adults that meet eligibility requirements by Indiana State Department of Health Guidelines:

  • Medicaid
  • Insurance does not cover vaccines
  • Uninsured
  • A current shot record is REQUIRED

Childhood lead poisoning prevention and screening for children ages 1 year to 6 years old

Immunization services are provided by appointment

Record Search $5.00 fee

FLU SHOTS
For information regarding flu shots and pre-qualification, please contact the Health & Human Services Department at 219-882-5565.
